How Do I Sign Up for Medicare Easy Pay?
To sign up for Medicare Easy Pay, you should complete and return the Authorization Agreement for Preauthorized Payments form (SF-5510). Information and documentation you will need include:
Basic info about you and your bank
Your Medicare card
A blank check from your bank account
Then, mail the completed form to:

It may take six to eight weeks to process your form.
How Do Automatic Bank Deductions Work?
Easy Pay deducts your Medicare premiums on the 20th of the month . The deduction occurs the next business day if the 20th falls on a weekend or holiday.
Medicare premium deductions will appear on your bank statement as a “CMS Medicare Premium ” automated clearing house (ACH) transaction. Your first Easy Pay deduction can include up to three months of premiums. Thereafter, CMS will never deduct more than one month’s premium plus $10.
If you owe less than $10, payment will not be deducted until the following month.
What Happens If My Easy Pay Payment Is Declined?
Typically, your Medicare premium is due on the 25th of the month, in the same month you get your bill. To ensure your payment is on time, Medicare recommends you submit the total amount due at least five business days before the due date.
If you owe more than three months of premiums, CMS won’t be able to deduct your premiums and you must pay them another way until the amount you owe is within the limits.
Your payment can also be declined for insufficient funds, and Easy Pay deductions may be interrupted . In this case, you must pay the full amount due using another payment method to resume Easy Pay deductions the following month.
How Can I Change or Stop Easy Pay?
You can change or stop automatic bank deductions by checking the box “CHANGE” or “STOP” on form SF-5510 .
When the Medicare premium changes, you don’t need to do anything. CMS will automatically deduct the new premium amount.
Other Ways to Pay
If you don’t wish to sign up for Easy Pay, you can:
Make a one-time payment with your credit or debit card or bank account at Medicare.gov .
Use your bank’s online bill pay.
Mail a check or money order with the coupon on your Medicare premium bill.
